Police dash cam footage captured the scream-inducing moment a FrontRunner train smashed into a FedEx delivery truck, amazingly leaving all involved passengers unharmed. The Utah Transit Authority (UTA) is now investigating why the Salt Lake City location's crossing gates were in an upward position at the time of the incident, the Salt Lake Tribunereported Tuesday.

"The agency has never had an accident like this before, and UTA is investigating why and how it happened to ensure it doesn't occur again," a UTA official said in a press release. The UTA's investigation, according to the Tribune, will encompass "all systems" and a series of interviews with employees aimed at pinpointing whether all procedures were properly followed prior to the collision. Safety remains the "top priority," the UTA added.

The collision, which occurred Saturday, was captured on a North Salt Lake cop's dash cam. The gates were up and the flashing signals usually used to alert drivers of an oncoming train were not activated, UTA officials said. A spokesperson for the agency later toldABC News that "a crew had been sent to the scene Saturday" to check on the crossing gates after reports of traffic problems caused by the crossing gates' safety default mode.

In a statement to KSTU, FedEx said they were able to work with local authorities after the incident to ensure minimal loss to consumers. "We are aware of the incident in Salt Lake City and are grateful that no one was seriously injured," a FedEx rep said. "We worked with authorities during their investigation to quickly clean up the scene and minimize the impact on customers."

The Utah Transit Authority did not immediately respond to Complex's request for comment.
